Upon completion of this symposium participants will be able to:

• Define and assess changes in healthcare delivery to improve the health of populations using strategies on three levels: healthcare industries, healthcare systems and individual providers.
• Analyze current and predicted changes in healthcare reimbursement, access, and policy that will shape emerging models of healthcare delivery.
• Evaluate priorities for preparing the healthcare workforce of the future.
• Integrate population health core principles and competencies into innovative educational programming.

Practice Gap:

The field of population health seeks to develop approaches to foster health and quality while managing cost and promoting wellness among diverse populations. A variety of population health strategies may be used by health systems to manage and effectively execute approaches to these challenges. In addition, new educational models may be necessary to train the healthcare workforce to effectively implement population health oriented practice.
